Requirement:

1. Fill in right TM Application Form and provide
2. Supporting documents accompanying the customer's application for subscription of the Service shall be required to be submitted to TM together with the Application Form. The required supporting documents are as follows:
   For Malaysian:
   1. Individual - a copy of the individual's Identity card (both side) and phone bill.
   2. Sole proprietor or partnership - a copy of either the Customer's business license, form D, B, or A;
   3. Company - a copy of Form 9, Form 49, Form 13.
3. Installation upon successful, customer will be charged the first 2 months usage*, one month pro-rated and the current month, which will appear in the first monthly bills.
4. The 1st bill will include stamp duty(*RM10), installation(*RM200), monthly subscription and pro-rate of monthly subscription fee.

*Not applicable during promotion period. Activation Fee and Stamp Duty Waived. Free Installation till further notice.
